The Rocky Mountain Regional Office and Agencies are seeking documentation to assist in preparing probate cases for submission to the Office of Hearings and Appeals.

We kindly ask all potential heirs of the probate cases to contact our offices by phone or email if you are able to assist.

Required Documents

Below is a list of documents we typically need:

  • Death Certificate or Informational Death Certificate (more cost-effective than a certified copy)
  • Marriage License
  • Divorce Decree
  • Adoption Order
  • Name Change Order
  • Birth Certificate
  • Wills and Testament
